This is a fully remote position, open to applicants in Florida.
• Develops, evaluates, and coordinates comprehensive care management activities for long-term care members.
• Creates or assists in formulating individualized service care plans/service plans.
• Informs members and their families/caregivers about available services, benefits, procedures, healthcare provider instructions, service options, referrals, and healthcare benefits.
• Assesses member needs and available resources, and suggests or facilitates plans for optimal outcomes.
• Formulates continuous long-term care plans/service plans.
• Identifies necessary providers, specialists, and community resources for long-term care.
• Coordinates communication between members/families/caregivers and care provider teams to ensure services are accessible.
• Offers resource support for employment, housing, participant direction, independent living, justice, and foster care requirements.
• Monitors care plans, member status, and outcomes, recommending updates based on member needs.
• Engages with long-term care healthcare providers and partners.
• Gathers, documents, and maintains member information and care management activities to ensure regulatory compliance.
• Conducts home and/or other site visits to assess member needs and collaborate with providers and partners.
• Provides leadership with feedback on opportunities for enhancing quality of care and cost-effective service delivery.
• Performs additional duties as assigned.
• Must reside in Miami Dade County, FL.
• Bachelor's degree with 2–4 years of relevant experience; or a Bachelor's degree in health, human, social work, or educational services with at least one year of qualifying experience; or a high school diploma or equivalent with three years of qualifying experience in case management for the elderly, including management of behavioral health conditions, individuals with physical or developmental disabilities, or the HIV/AIDS population.
• Experience in interviewing and evaluating member needs.
• Knowledge and experience concerning caseload management and casework practices.
• Familiarity with determining eligibility for DHSS programs.
• Understanding of Federal and State laws as they pertain to DHSS programs.
• Ability to effectively solve problems and locate community resources.
• Capacity to collaborate with caregivers, involved State agency representatives, and providers.
• Strong interpersonal skills.
• Ability to practice Cultural Competency with awareness and respect for diversity.
• Knowledge of the needs and service delivery system for all populations within the case manager’s caseload.
• Field-based work.
• Technologically proficient.
• Exceptional customer service skills.
• Experience in hospice care.
• Background in the Department of Children and Families.
• Outstanding written and verbal communication skills.
• Experience in case management.
• Bilingual (Spanish).
