This is a fully remote position, open to applicants in New York.
• Conduct monthly care management discussions via outbound calls to designated patients to address their healthcare requirements.
• Relay information about resources and services accessible to patients throughout their care journey.
• Recognize patient-specific issues, objectives, and interventions tailored to fulfill the individual needs of patients through actionable and time-sensitive goals.
• Ensure compliance with documentation standards, including but not limited to medical history, medications, immunizations, allergies, surgical history, and family background.
• Exhibit understanding of a patient’s situation that may require adjustments to care plans, goals, and preferences, while being cognizant of barriers to care and services.
• Provide relevant health education.
• Refer patient concerns to the triage nurse team as necessary.
• Maintain performance metrics, ensuring proper utilization.
• Attain the scheduled call percentage.
• Achieve quality scores.
• Adhere to scheduled hours and maintain consistent attendance.
• A current active license in NY State and an active multistate license.
• Proficiency in adhering to established clinical protocols and procedures.
• Ability to adapt and switch tasks efficiently.
• Willingness to support colleagues and contribute to the overall success of the team.
