LPN / LVN – Nurse

This is a fully remote position, open to applicants in United States.

📋 Description

• Facilitate the enrollment of patients into our Remote Therapeutic Monitoring (RTM) or Chronic Care Management (CCM) programs utilizing Flagler’s portal.

• Conduct daily outreach through calls and text messages to patients to track their health status and ensure engagement.

• Aid in the collection and documentation of patient health information, which includes medical histories, current health status, and any new symptoms.

• Provide support to patients by sending health tips, such as stretching routines, meditation videos, and light exercises.

• Uphold strict confidentiality when handling patient information, in compliance with HIPAA regulations and company policies.

• Engage in ongoing training and regular check-ins as part of the company's professional development efforts.

⛳️ Requirements

• Valid certification as a Licensed Practical Nurse (LPN).

• Over 3 years of experience in either an office or remote work environment.

• Proficient with technology tools including Zoom, Slack, and online enrollment platforms.

• Strong organizational abilities with a knack for efficiently managing multiple tasks.

• Exceptional communication skills, both written and verbal, emphasizing empathy and patient care.

• Capability to work autonomously in a remote setting, demonstrating initiative and self-motivation.

• Knowledge of HIPAA regulations with a dedication to preserving patient privacy.
