
LPN Care Coordinator – CCM/RPM

This is a fully remote position, open to applicants in United States.
• Provide care coordination services directly to patients of Alopex clients.
• Oversee and coordinate care for individuals enrolled in Alopex’s services.
• Collaborate closely with patients, primary care providers, and specialists to create, implement, and monitor personalized care plans.
• Ensure that patients receive top-notch care through continuous communication and coordination.
• Conduct a thorough review of electronic medical records (EMR).
• Create and maintain tailored Care Plans for patient management.
• Implement and oversee all care management activities related to patients throughout the continuum of care.
• Communicate effectively with the patient’s clinical care team.
• Track patient progress toward desired outcomes through assessment and evaluation.
• Keep accurate and comprehensive documentation related to medications, issues, goals, interventions, preventative care, and more.
• Perform monthly care calls to patients enrolled in the program.
• Address all patient needs and concerns, including necessary follow-ups.
• Educate patients about chronic conditions and reinforce the provider’s instructions regarding these conditions.
• Assess and address social determinants of health and begin to connect patients with community resources, subsequently entering this information into the system.
• Evaluate and assess medical conditions, identifying and addressing changes in a clinically appropriate manner.
• Comply with all policies and procedures as outlined by Medicare and Shifox/Alopex.
• Bilingual in English and Spanish is required.
• A minimum of two (2) years’ experience in providing patient-centered care is necessary.
• At least one (1) year of experience in skilled nursing is required.
• Experience with electronic medical records is essential.
• Ability to perform nursing assessments, identify problems, and develop care plans.
• Adaptable to new technology with experience in computer documentation.
• Excellent time management skills are a must.
• Capability to assess, evaluate, and address patients' conditions and concerns effectively.
• Strong verbal and written communication skills are essential.
