This is a fully remote position, open to applicants in Washington.
• Evaluates, plans, and implements care strategies that are tailored to each member and aimed at achieving the most suitable and least restrictive level of care.
• Works collaboratively with members, families, and healthcare providers to create a personalized care plan.
• Facilitates individual counseling and group therapy sessions for adolescents and adults.
• Identifies and initiates referrals to social service programs, including financial, psycho-social, community, and state support services.
• Advocates for members and their families as necessary to ensure that the patient's needs and preferences are adequately represented and supported by the healthcare team.
• Applies approved clinical criteria to evaluate and determine the appropriate level of care for members.
• Records all member assessments, care plans, and referrals provided.
• Accountable for meeting established goals and Key Performance Indicators (KPIs).
• Engages with the StrongWell model, taking responsibility and ownership for outcome-based care.
• Participates in interdisciplinary team meetings and utilization management rounds, contributing information to facilitate safe transitions of care.
• Advocates for responsible and ethical management of company resources.
• Maintains a record of excellent punctuality and attendance during work hours.
• Must possess LCSW, LMFT, LMHC, or LPC credentials in Washington.
• Preferred experience with populations dealing with substance abuse.
• Background in maternal and substance abuse is preferred.
• Experience working with both adults and adolescents.
• Knowledge of Medicare and Medicaid procedures is required.
• Experience with remote work is a plus.
