This is a fully remote position, open to applicants in Illinois, +3 more states.
• Leverage our specialized care management software to reach out to a full caseload of Medicare patients who have two or more chronic conditions on a monthly basis.
• Establish and nurture relationships with patients to guide them towards better health through SMART goals and education on self-management techniques.
• Enhance and update the Plan of Care by revising medications, upcoming appointments, biometrics, symptoms, and implemented interventions.
• Link patients to community resources as necessary, including transportation, personal care, prescription/DME assistance, and social services.
• Perform Transitional Care Management activities for high-risk patients recently discharged from the hospital to minimize unnecessary readmissions.
• Address care gaps by promoting preventive care initiatives, such as annual well visits, vaccinations, cancer screenings, and follow-up appointments.
• Proficient in English.
• Self-motivated and capable of working independently with minimal supervision while achieving performance targets.
• Strong enthusiasm for nursing and enhancing patient outcomes.
• Technologically savvy and willing to learn and utilize new software applications.
• Exceptional organizational and time management abilities.
• Excellent communication and telephone skills.
• Robust critical thinking and problem-solving capabilities.
• A current, unrestricted Illinois RN license is mandatory.
• Experience with EHRs and web-based applications is essential.
• A minimum of 3 years' experience as a Registered Nurse is required.
• Experience in Case Management or Chronic Disease Management is highly preferred.
• Certification as a Diabetes Educator is desirable but not essential.
• Familiarity with Motivational Interviewing or other behavior change communication techniques is advantageous.
• Compensation is provided at a rate of $15.00 per initial clinical encounter per patient per month.
• Your initial payment will be issued monthly via direct deposit, 40 days following the last day of the month of service.
• All subsequent payments after the first will be deposited approximately 30 days after the last day of the month of service.
