Licensed Practical Nurse, Principal Care Management

This is a fully remote position, open to applicants in United States.

📋 Description

• Evaluate patient charts, medications, laboratory results, and care plans.

• Facilitate care coordination with providers, nursing personnel, pharmacies, and families as necessary.

• Accurately and thoroughly document patient interactions.

• Recognize changes in patient conditions and escalate concerns as needed.

• Provide assistance in chronic disease management and patient education.

• Ensure that PCM services are performed in accordance with CMS guidelines.

⛳️ Requirements

• Current LPN license is mandatory.

• Experience in nursing homes or long-term care is preferred.

• Background in chronic care management or PCM is advantageous.

• Excellent communication and organizational abilities.

• Ability to work independently in a remote setting.

• EMR experience is preferred.
