Licensed Practical Nurse (LPN) – Principal Care Management

This is a fully remote position, open to applicants in United States.

📋 Description

• Evaluate patient charts, medications, laboratory results, and care plans.

• Facilitate care coordination with healthcare providers, nursing personnel, pharmacies, and families as necessary.

• Accurately and comprehensively document patient interactions.

• Recognize changes in patient conditions and appropriately escalate any concerns.

• Support chronic disease management and provide patient education.

• Ensure that PCM services are conducted in accordance with CMS regulations.

⛳️ Requirements

• A valid LPN license is mandatory.

• Experience in nursing homes or long-term care settings is preferred.

• Familiarity with chronic care management or PCM is advantageous.

• Excellent communication and organizational abilities are essential.

• Ability to work independently in a remote setting is required.

• Experience with electronic medical records (EMR) is preferred.
