This is a fully remote position, open to applicants in Wyoming.
β’ Oversee patient census with a focus on resolving issues to bridge gaps in both clinical and non-clinical patient care.
β’ Identify and connect patients with community resources that may enhance their care.
β’ Deliver patient education and promote health literacy regarding the management of chronic illnesses.
β’ Conduct medication management, which includes recognizing potential medication issues, performing reconciliation, ensuring adherence, and coordinating refills.
β’ Support the timely provision of services for your patients, including Home Health, DME, Home Infusion, and other essential needs.
β’ Address patient inquiries and foster open communication to better understand their needs.
β’ Assist with managing referrals and scheduling appointments.
β’ Current Compact/Multi-State license (LPN) (LVN)
β’ Proficient in Microsoft Office Suite
β’ Strong written and verbal communication abilities
β’ Experience with Epic (desirable)
β’ Bilingual skills (desirable)
β’ Additional Single State licenses (desirable)
