Licensed Practical Nurse, LPN, LVN

This is a fully remote position, open to applicants in Wyoming.

📋 Description

• Oversee patient census with a focus on resolving issues to bridge gaps in both clinical and non-clinical patient care.

• Identify and connect patients with community resources that may enhance their care.

• Deliver patient education and promote health literacy regarding the management of chronic illnesses.

• Conduct medication management, which includes recognizing potential medication issues, performing reconciliation, ensuring adherence, and coordinating refills.

• Support the timely provision of services for your patients, including Home Health, DME, Home Infusion, and other essential needs.

• Address patient inquiries and foster open communication to better understand their needs.

• Assist with managing referrals and scheduling appointments.

⛳️ Requirements

• Current Compact/Multi-State license (LPN) (LVN)

• Proficient in Microsoft Office Suite

• Strong written and verbal communication abilities

• Experience with Epic (desirable)

• Bilingual skills (desirable)

• Additional Single State licenses (desirable)
