This is a fully remote position, open to applicants in United States.
• Oversee patient census with a focus on solutions to bridge gaps in both clinical and non-clinical patient care.
• Identify and facilitate access to community resources that can enhance patient care.
• Educate patients and improve health literacy regarding the management of chronic conditions.
• Conduct medication management, which includes recognizing potential medication issues, reconciliation, ensuring adherence, and coordinating refills.
• Help to ensure the timely provision of services to patients, including Home Health, DME, Home Infusion, and other essential needs.
• Address patient inquiries and foster open communication to better understand their needs.
• Assist with managing referrals and scheduling appointments.
• Active Multi-State/Compact License (LPN) (NCL) (LVN)
• Proficient in Microsoft Office Suite
• Strong written and verbal communication skills
• Experience with Epic (preferred)
• Bilingual skills (preferred)
• Additional single state licenses (LPN) (preferred)
