This is a fully remote position, open to applicants in Tennessee.
• Oversee patient census with a focus on resolving issues to enhance both clinical and non-clinical patient care.
• Identify and connect patients with community resources that can improve their care.
• Deliver patient education and enhance health literacy regarding the management of chronic conditions.
• Conduct medication management, which includes identifying potential medication issues, reconciliation, adherence, and coordinating refills.
• Aid in ensuring timely service delivery for your patients, including Home Health, DME, Home Infusion, and other essential needs.
• Address patients' inquiries and foster an open dialogue to better understand their needs.
• Assist in managing referrals and scheduling appointments.
• Active Multi-State/Compact License (LPN) (NLC) (LVN)
• Proficient in Microsoft Office Suite
• Strong written and verbal communication skills
• Minimum of 3 years of direct patient-facing work experience
• Experience with Epic (preferred)
• Bilingual ability (preferred)
• Additional single state licensures (LPN) (preferred)
