Licensed Practical Nurse, LPN, LVN

This is a fully remote position, open to applicants in South Carolina.

📋 Description

• Oversee patient census using a solution-oriented approach to address gaps in both clinical and non-clinical patient care.

• Identify and coordinate community resources that would enhance patient care.

• Educate patients on health literacy and the management of chronic conditions.

• Conduct medication management, which includes identifying potential medication issues, reconciliation, adherence, and coordinating refills.

• Support the timely delivery of essential services to patients, including Home Health, DME, Home Infusion, and other critical needs.

• Address patients' inquiries and foster open communication to better understand their needs.

• Assist with managing referrals and scheduling appointments.

⛳️ Requirements

• Active Multi-State/Compact License (LPN) (NLC) (LVN)

• Proficient in Microsoft Office Suite

• Strong written and verbal communication skills

• Experience with Epic (preferred)

• Bilingual skills (preferred)

• Additional single state licensures (LPN) (preferred)
