Licensed Practical Nurse, LPN, LVN

This is a fully remote position, open to applicants in Oklahoma.

📋 Description

• Oversee patient census with a focus on resolving issues to enhance both clinical and non-clinical patient care.

• Identify and connect patients with community resources that can support their care needs.

• Educate patients on health literacy and the management of chronic illnesses.

• Conduct medication management, which includes identifying potential medication issues, reconciliation, adherence, and coordinating refills.

• Help ensure prompt delivery of services to your patients, including Home Health, DME, Home Infusion, and other essential needs.

• Address patient inquiries and foster an open dialogue to better understand their requirements.

• Assist in managing referrals and scheduling appointments.

⛳️ Requirements

• Active Multi-State/Compact License (LPN) (NLC) (LVN)

• Proficient in Microsoft Office Suite

• Strong written and verbal communication abilities

• Experience with Epic (preferred)

• Bilingual skills (preferred)

• Additional single state licensure (LPN) (preferred)
