Licensed Practical Nurse, LPN, LVN

This is a fully remote position, open to applicants in Kansas.

📋 Description

• Oversee patient census with a focus on resolving issues to enhance both clinical and non-clinical aspects of patient care.

• Identify and connect patients with community resources that could improve their care experience.

• Educate patients on health literacy and the management of chronic conditions.

• Conduct medication management tasks, including recognizing potential medication issues, reconciliation, adherence, and coordinating refills.

• Support the timely provision of services for your patients, including Home Health, DME, Home Infusion, and other essential needs.

• Address patients' inquiries and foster open communication to better understand their requirements.

• Assist in managing referrals and scheduling appointments.

⛳️ Requirements

• Active Multi-State/Compact License (LPN) (NLC) (LVN)

• Proficient in Microsoft Office Suite

• Exceptional written and verbal communication abilities

• Experience with Epic (preferred)

• Bilingual skills (preferred)

• Additional single state licenses (LPN) (preferred)
