Licensed Practical Nurse, LPN, LVN

This is a fully remote position, open to applicants in Florida.

📋 Description

• Oversee patient census utilizing a resolution-focused approach to address gaps in both clinical and non-clinical patient care.

• Identify and connect patients with community resources that can enhance their care.

• Educate patients and improve health literacy regarding the management of chronic conditions.

• Conduct medication management, which includes identifying potential medication issues, reconciliation, adherence, and coordinating refills.

• Aid in the timely delivery of services to patients, including Home Health, DME, Home Infusion, and other essential needs.

• Address patients' inquiries and foster an open dialogue to better understand their needs.

• Assist in managing referrals and scheduling appointments.

⛳️ Requirements

• Active Multi-State/Compact License (LPN) (NLC) (LVN)

• Proficiency in Microsoft Office Suite

• Exceptional written and verbal communication skills

• Minimum of 3 years of experience in direct patient-facing roles
