Licensed Practical Nurse, LPN, LVN

This is a fully remote position, open to applicants in Alabama.

📋 Description

• Oversee patient census while employing a resolution-focused strategy to address gaps in both clinical and non-clinical patient care.

• Identify and connect patients with community resources that would enhance their care.

• Deliver patient education and enhance health literacy regarding the management of chronic conditions.

• Conduct medication management, which includes recognizing potential medication issues, reconciliation, adherence, and coordinating refills.

• Aid in ensuring the prompt delivery of services to patients, including Home Health, DME, Home Infusion, and other essential needs.

• Address patient inquiries and foster open communication to comprehend their needs.

• Assist with managing referrals and scheduling appointments.

⛳️ Requirements

• Current Multi-State/Compact License (LPN) (NCL) (LVN)

• Proficiency in Microsoft Office Suite

• Strong written and verbal communication skills

• Experience with Epic (Preferred)

• Bilingual skills (Preferred)

• Additional single state licenses (LPN) (Preferred)
