Licensed Physical Therapy Assistant, Care Navigator

This is a fully remote position, open to applicants in Minnesota.

📋 Description

• Deliver remote monitoring services to assist patients as they navigate their home-exercise therapy programs.

• Guide and support users in therapeutic exercises, neuromuscular re-education, and functional training via our technology platform.

• Offer continuous feedback to the evaluating clinician regarding patient progress beyond the clinic.

• Maintain essential records and documentation while conducting remote monitoring.

• Contribute to enhancing the processes and functionality of the platform.

• Organize and manage a caseload efficiently.

⛳️ Requirements

• A minimum of 2 years of experience in musculoskeletal health is advantageous.

• Must have graduated from an accredited physical therapy program.

• Possess an active PTA license in Minnesota.

• Demonstrate tech-savviness - excel in a multi-platform environment, seamlessly switching between systems (EHR, messaging, scheduling) without missing a beat.

• Experience in behavioral modification; Health Coach Certification is a plus.

• Exhibit excellent communication, interpersonal, and presentation skills, along with the ability to gather and present significant data to inform decisions.

• Strong relationship-building and listening skills are essential.

• Show a demonstrated patient focus with a genuine passion for user experience.

• Capability to multitask and manage diverse types of users effectively.

• Ability to work autonomously with minimal supervision.

• Flexibility and agility to collaborate across functions in a dynamic environment.

• Capacity to influence change while bringing enthusiasm and excitement to the workplace.
