Licensed Physical Therapist, Care Navigator

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Deliver remote monitoring services to assist patients as they advance through their home-exercise therapy program.

• Guide and support users in therapeutic exercise, neuromuscular re-education, and functional training via our technology platform.

• Continuously provide feedback to the evaluating clinician regarding patient progress outside of the clinical setting.

• Maintain essential records and documentation while conducting remote monitoring.

• Contribute to enhancing processes and functionality of the platform.

• Organize and oversee a caseload while ensuring efficiency.

⛳️ Requirements

• A minimum of 2 years of experience in musculoskeletal health is advantageous.

• Must have graduated from an accredited physical therapy program.

• Possession of an active California PT license is required.

• Proficient with technology – excels in a multi-platform environment.

• Experience in behavioral modification; Health Coach Certification is a plus.

• Outstanding communication, interpersonal, and presentation skills.

• Strong ability to build relationships and listen effectively.

• Proven patient-centered approach.

• Capability to multi-task and manage diverse types of users.

• Ability to work independently with minimal supervision.

• Flexibility and adaptability to collaborate across functions in a dynamic environment.

• Capacity to influence change and bring enthusiasm and energy to the workplace.
