Licensed Clinical Social Worker – EST

This is a fully remote position, open to applicants in United States.

📋 Description

• Work collaboratively with a multidisciplinary clinical team to provide comprehensive, patient-focused palliative care for individuals with serious illnesses.

• Perform thorough assessments that evaluate psychosocial distress, mental health requirements, caregiver strain, social determinants of health, and risk factors affecting patients with serious illnesses.

• Analyze assessment and screening outcomes to pinpoint priority needs, emerging risks, and suitable interventions within the palliative care framework.

• Engage in serious illness discussions with patients and their families, facilitating advance care planning dialogues and documentation.

• Provide clinical recommendations and escalate issues to physicians, advanced practice providers, and other clinicians in line with established protocols, particularly concerning safety, mental health, caregiver stress, or complex psychosocial challenges.

• Create, document, and contribute clinically relevant interventions within personalized care plans, ensuring they align with patient goals, values, and value-based care outcomes.

• Take part in interdisciplinary rounds and case conferences, offering clinical insights that support coordinated, goal-oriented care and enhance patient and family experiences.

• Offer short-term clinical counseling, psychoeducation, and emotional support to patients and caregivers managing serious illness, disease progression, and end-of-life issues, within the limits of LCSW licensure.

• Act as a clinical resource for the care team by identifying psychosocial obstacles to care, recommending evidence-based interventions, and aiding transitions across different care settings.

• Use Tuesday Health’s electronic systems, tools, and technologies to document clinical assessments, interventions, and care plans, facilitating coordinated care delivery and quality measurement in a value-based setting.

• Build and sustain therapeutic, trust-based relationships with patients and families, rooted in clinical listening, empathy, and culturally sensitive care.

• Collaborate with the multidisciplinary team to enhance quality of life, minimize avoidable utilization, and support holistic outcomes, including timely and appropriate hospice transitions for members with serious illnesses.

• Support the interdisciplinary team through debriefings after emotionally intense cases and lead periodic memorial rounds for the team.

⛳️ Requirements

• Master’s in Social Work (MSW) from an accredited institution.

• Active, unrestricted Licensed Clinical Social Worker (LCSW) license in the relevant state(s).

• Strong clinical experience in palliative care is essential, including familiarity with advance care planning and serious illness discussions.

• Experience in an interdisciplinary care model, collaborating with physicians, advanced practice providers, nurses, care managers, health plan partners, and community resources.

• Proven ability to exercise independent clinical judgment, including knowing when and how to escalate concerns appropriately or respond to urgent or emergent situations.

• Excellent verbal and written communication skills, with the ability to document clinical assessments, interventions, and care plans clearly.

• Strong organizational and time-management abilities, capable of juggling multiple priorities while ensuring a high-quality patient and family experience.

• Proficiency in electronic health records (EHRs), clinical documentation systems, and digital tools, with an aptitude for quickly adapting to new technologies as the organization evolves.

• Ability to thrive in a fast-paced, changing environment, demonstrating flexibility, resilience, and comfort with ambiguity.
