This is a fully remote position, open to applicants in United States.
• Conduct comprehensive psychosocial and social determinants of health assessments that encompass housing, food security, financial status, transportation, safety, caregiver strain, and social isolation.
• Develop member-focused plans with specific, achievable goals and revise them as situations evolve.
• Utilize standardized tools to screen for issues such as depression, anxiety, cognitive concerns, and safety risks.
• Assist members in applying for and accessing Medicaid, Medicare Savings Programs, SNAP, and utility or housing assistance.
• Link members to community resources, including Area Agencies on Aging, meal programs, transportation services, and in-home support.
• Follow up to ensure that connections have been made and resources are functioning effectively.
• Offer brief, solution-oriented support within the scope of practice and coordinate referrals to behavioral health providers when ongoing care is necessary.
• Provide support to family caregivers through education, planning, and connections to respite and support resources.
• Recognize crisis situations and promptly follow escalation protocols, including same-day routing of safety concerns.
• Accurately document each member interaction within established timeframes.
• Adhere to consent, HIPAA privacy, and mandated reporting standards.
• Drive connections to member resources and benefits, as well as member/caregiver-reported experiences.
• Maintain thorough, accurate, and timely documentation, escalating safety and crisis issues on the same day.
• Conduct all member interactions via phone or video; ongoing psychotherapy is not included.
• Active, unrestricted LCSW license in at least one U.S. state.
• Master of Social Work (MSW) degree from a CSWE-accredited program.
• Residing in the United States.
• A minimum of 2 years of post-master's experience in medical, community, geriatric, or healthcare social work.
• Strong understanding of public benefits and community resources.
• Proven ability to establish trust with older adults and caregivers.
• Proficiency with care management platforms and phone/video communication tools.
• Self-motivated with the ability to manage your own member caseload and schedule independently.
• Access to a reliable computer, high-speed internet, and a private, HIPAA-compliant workspace.
• Experience with Medicare, dual-eligible, or older adult populations is a plus.
• Background in hospice, palliative care, dementia, or caregiver support is advantageous.
• Licensure in multiple states is considered beneficial.
• Bilingual skills are preferred, especially in Spanish.
