
Lead Telehealth Physician

This is a fully remote position, open to applicants in Arizona, +7 more states.
• Perform initial telehealth consultations with enrolled members to evaluate medical requirements, identify care gaps, and address barriers to accessing care.
• Build and nurture a high-performing clinical team by providing mentorship and opportunities for professional development to promote long-term growth and leadership within the organization.
• Create clear clinical strategies and care priorities, collaborating with the care team to ensure seamless transitions.
• Oversee clinical quality assurance: Review clinical documentation to verify compliance with community health billing and program standards, ensuring that the sequencing of social determinants is logical and actionable.
• Manage physician training: Act as the primary clinical contact for our physician team — onboard new clinicians, provide constructive feedback, lead retraining sessions, and ensure alignment with company values.
• Manage appointments and workflows: Assist in organizing scheduling, intake, SMA logistics, and clinical workflows to scale operations efficiently across different markets.
• Policy improvement: Identify ineffective processes in the field and promptly revise clinical policies, intake forms, and care plan templates.
• Metric oversight: Monitor and report on essential clinical KPIs, including ER diversion rates, care gap closures, and resolutions of SDoH.
• Collaborate with Area Managers and LCSWs to connect medical providers with Community Health Workers, ensuring the team functions as a cohesive care engine.
• Offer clinical insights to enhance program workflows as the organization expands.
• MD or DO with active, unrestricted licensure in multiple states (required: Pennsylvania, California, Arizona, Texas, South Carolina, New Mexico, New Jersey, Tennessee; additional state licenses highly preferred).
• Board certification in Family Medicine, Internal Medicine, or a related primary care specialty.
• Experience in managing or supervising other clinicians (MD/DO/NP/PA), including providing constructive feedback.
• Profound understanding of community health and Social Determinants of Health (SDoH).
• Background in group visit models is strongly preferred.
• Proven experience using data to enhance clinical quality and auditing documentation for compliance purposes.
• Comfortable working with Medicare, Medicaid, and complex patient populations.
• Telehealth experience is required.

• Tech-savvy: easily navigates EHRs, Slack, and Google Suite, and can train others on these platforms.
