This is a fully remote position, open to applicants in California.
• Oversee a designated patient panel for Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) enrollment and engagement, consistently achieving or surpassing outreach, conversion, and monthly patient interaction targets.
• Ensure timely, accurate, and compliant documentation in line with Welby Health policies, clinical protocols, and relevant program billing requirements.
• Exemplify the high standards of outreach quality, professionalism, and patient-centered service that are expected of all Medical Assistants.
• Track daily productivity of Medical Assistants regarding outreach activities, enrollment success, and documentation quality, proactively spotting performance trends and implementing corrective measures when metrics fall short.
• Facilitate regular one-on-one coaching sessions with each Medical Assistant, providing performance feedback that aligns directly with operational data, key performance indicators, and program results.
• Oversee the distribution of patient caseloads to maximize team capacity and operational efficiency, escalating workload issues or performance challenges to clinical leadership with documented action plans.
• Lead the onboarding and training process for all newly hired Medical Assistants, ensuring their successful integration into the team and attainment of expected productivity levels within the first thirty (30) days of employment.
• Create, maintain, and continually improve Standard Operating Procedures (SOPs), training materials, and reference resources that support Medical Assistant workflows and documentation standards.
• Identify opportunities for individual and team development and design targeted training programs to proactively address skill gaps and enhance overall team performance.
• Collaborate with Human Resources during the recruitment process to establish hiring criteria, assess candidates, and offer hiring recommendations based on qualifications, competencies, and cultural fit.
• Anticipate staffing needs and communicate resource requirements to facilitate timely recruitment efforts before operational gaps affect performance.
• Perform periodic audits of documentation to ensure adherence to organizational standards and provide structured, actionable feedback that promotes sustained quality improvement.
• Monitor team key performance indicators (KPIs) through performance dashboards and integrate data-driven insights into coaching discussions, performance evaluations, and team meetings.
• Ensure all Medical Assistants stay updated on Welby Health clinical protocols, payer requirements, regulatory standards, and program compliance benchmarks.
• Evaluate operational and clinical performance metrics to identify trends, assess team performance, and implement data-driven strategies that enhance patient engagement, productivity, quality outcomes, and overall program performance.
• Associate’s degree in a healthcare-related field.
• Active and valid medical assistant (MA) certification in good standing.
• Approximately five (5) or more years of direct patient care experience in medical-surgical or home care.
• Outstanding written and verbal communication skills.
• Ability to work independently in a remote, technology-enabled environment.
• Strong organizational skills with a keen attention to detail and follow-through.
• Proven capability to interpret performance dashboards, analyze operational metrics, and hold team members accountable to established performance standards.
