This is a fully remote position, open to applicants in California.
• The Lead ALF Case Manager is tasked with ensuring the quality of life and overall well-being of designated ALWP and CCT clients within a specific region of RCFEs.
• This will involve conducting monthly in-person visits with each client, along with weekly follow-up visits with clients, their social worker, or their RN to confirm their continued well-being in the RCFE.
• The Lead ALF Case Manager will assess the services required for each client and collaborate with the CCT and/or ALWP coordinators, the CCT Manager, and the ALW/ALF Manager to guarantee that these services are delivered.
• The Lead ALF Case Manager will foster relationships with the staff at each RCFE and strive to ensure that Libertana maintains effective communication with them.
• Quality assurance reviews may be conducted as necessary.
• The Lead ALF Case Manager will also contribute to the development of PR strategies, oversee client databases, and assist the Director of Waiver Services with overall program development.
• A minimum of 2 years of experience in a related field is required.
• A bachelor's degree in social work, psychology, counseling, rehabilitation, gerontology, sociology, or a related discipline is essential.
• Current CPR certification is necessary, or the ability to obtain it upon hiring.
• Familiarity with California Community Transitions and California Assisted Living Waiver Programs is preferred.
• Must demonstrate exceptional judgment and possess a strong understanding of clinical practices and documentation skills.
• Excellent organizational abilities and the capacity to work independently are required.
• Strong written and verbal communication skills are essential.
• Skills in application management and public relations are necessary.
• Proficiency in using computers and electronic information and charting systems is required.
• Ability to establish and maintain positive relationships with Libertana office personnel and RCFE staff is crucial.
• A valid driver's license and a well-maintained vehicle are required.
