Intake Coordinator – Licensed Practical Nurse (LPN)

This is a fully remote position, open to applicants in United States.

📋 Description

• Handling transfers from the Patient Enrollment team to perform initial health assessments for newly enrolled patients within our network.

• Addressing patient inquiries and fostering an open dialogue to better understand their needs.

• Identifying and coordinating community resources that can enhance patient care.

• Offering patient education and promoting health literacy regarding chronic condition management.

• Supporting medication management by identifying potential medication issues, ensuring adherence, and coordinating refills.

• Assisting in the timely delivery of services for your patients, including Home Health, DME, Home Infusion, and other essential needs.

• This position demands the ability to swiftly adjust to a dynamic work environment, with a significant portion of your shift spent on the phone.

• Active Compact/Multi-State license (LPN) (LVN)

• Technical proficiency in Microsoft Office Suite

• Strong written and verbal communication abilities
