Intake Coordinator – Licensed Practical Nurse (LPN)

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Conduct preliminary health assessments for newly enrolled patients in our network, accepting transfers from the Patient Enrollment team.

• Address patients' inquiries and foster an open dialogue to comprehend their needs.

• Identify and coordinate community resources that would enhance patient care.

• Educate patients on health literacy and the management of chronic conditions.

• Support medication management by identifying potential medication issues, ensuring adherence, and coordinating refills.

• Ensure timely delivery of essential services to patients, including Home Health, DME, Home Infusion, and other critical needs.

• The individual must be able to adapt swiftly to a fast-paced work environment, as this role requires spending most of the shift on the phone.

• Possession of an Active Compact/Multi-State license (LPN) (LVN).

• Technical proficiency in Microsoft Office Suite.

• Outstanding written and verbal communication abilities.
