This is a fully remote position, open to applicants in Oklahoma.
• CareHarmony’s Intake Coordinators (LPN) (NLC) (LVN) collaborate closely with healthcare providers to implement value-based care management strategies for patients.
• We are looking for a skilled Licensed Practical Nurse – LPN Nurse (LPN) (NLC) (LVN) who has a minimum of 3 years of hands-on patient care experience, excels in a dynamic environment, is self-driven, possesses exceptional attention to detail, and appreciates the role they play in enhancing a patient’s healthcare experience.
• Experience in recognizing resources and coordinating requirements for chronic care management patients is essential.
• This position is ideal for individuals who enjoy meeting and helping new patients on a daily basis.
• Full-Time availability is required from Monday to Friday, with no weekend shifts and an average of one rotational on-call duty per year.
• Many of our team members experience accelerated advancement within the company.
• We welcome transfers from the Patient Enrollment team to carry out preliminary health evaluations for newly enrolled patients in our network.
• Address patient inquiries while fostering an open dialogue to better understand their needs.
• Identify and connect patients with community resources that could enhance their care.
• Provide education to patients regarding health literacy and the management of chronic conditions.
• Assist with medication management, which includes identifying potential medication issues, ensuring adherence, and coordinating refills.
• Help guarantee the timely delivery of services to your patients, such as Home Health, DME, Home Infusion, and other critical needs.
• The candidate must be capable of swiftly adjusting to a fast-paced work setting, as this role requires substantial phone interaction throughout the shift.
• Active Compact/Multi-State license (LPN) (LVN) is mandatory.
• Proficient technical skills, particularly with the Microsoft Office Suite, are required.
• Outstanding written and verbal communication abilities are essential.
• Must have an active high-speed Wi-Fi connection.
• A home office or HIPAA-compliant workspace is necessary.
• Experience with Epic is preferred.
• Bilingual candidates are encouraged to apply.
• Additional Single State licenses may be beneficial.
