Intake Coordinator – Licensed Practical Nurse (LPN)

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Receiving transfers from the Patient Enrollment team to perform initial health evaluations for newly registered patients within our network.

• Addressing patients' inquiries and fostering open communication to comprehend their needs.

• Recognizing and coordinating community resources that may enhance patient care.

• Offering education to patients and improving health literacy regarding the management of chronic conditions.

• Supporting medication management by identifying potential medication issues, promoting adherence, and organizing refills.

• Aiding in the timely provision of services to your patients, including Home Health, DME, Home Infusion, and other essential needs.

• The individual must be capable of swiftly adapting to a dynamic work environment. This position involves spending the majority of your shift on the phone.

⛳️ Requirements

• Active Compact/Multi-State license (LPN) (LVN)

• Technical proficiency in Microsoft Office Suite

• Outstanding written and verbal communication abilities

• Must possess active high-speed Wi-Fi

• Must have a home office or a workspace that complies with HIPAA regulations
