Intake Coordinator – Licensed Practical Nurse (LPN)

This is a fully remote position, open to applicants in New Mexico.

📋 Description

• CareHarmony’s Intake Coordinators (LPN) (NLC) (LVN) collaborate extensively with providers to implement value-based care management initiatives for their patients.

• CareHarmony is on the lookout for a skilled Licensed Practical Nurse – LPN Nurse (LPN) (NLC) (LVN) with a minimum of 3 years of direct patient-facing experience, who excels in a fast-paced environment, is self-driven, possesses outstanding attention to detail, and acknowledges the difference they can make in a patient’s healthcare journey.

• You should have experience in identifying resources and coordinating the needs of chronic care management patients. This position is ideal for individuals who enjoy meeting and assisting new patients on a daily basis.

⛳️ Requirements

• Accept transfers from the Patient Enrollment team to carry out preliminary health assessments for newly enrolled patients in our network.

• Address patients' inquiries and foster open communication to comprehend their needs.

• Identify and coordinate community resources that could enhance patient care.

• Provide education to patients on health literacy and the management of chronic conditions.

• Aid in medication management, including spotting potential medication issues, adherence, and coordinating refills.

• Assist in ensuring the timely delivery of services to your patients, including Home Health, DME, Home Infusion, and other essential needs.

• The candidate must be capable of quickly adapting to a dynamic work environment, with most of your shift spent on the phone.

• Active Compact/Multi-State license (LPN) (LVN).

• Proficient in Microsoft Office Suite.

• Exceptional written and verbal communication abilities.

• Must have an active high-speed Wi-Fi connection.

• Must have a home office or a HIPAA-compliant workspace.

• Experience with Epic.

• Bilingual skills are a plus.

• Additional Single State licensures are beneficial.
