📋 Description

• The Intake Coordinators at CareHarmony (LPN) (NLC) (LVN) collaborate extensively with healthcare providers to implement value-based care management strategies for patients.

• CareHarmony is in search of a skilled Licensed Practical Nurse – LPN (NLC) (LVN) who possesses a minimum of 3 years of hands-on patient interaction experience, thrives in a dynamic setting, is self-driven, exhibits exceptional attention to detail, and appreciates the influence they can have on a patient's healthcare experience.

• You should have expertise in identifying resources and managing the needs of chronic care management patients. This position is ideal for individuals who enjoy meeting and assisting new patients every day.

• This is a fully remote role, allowing you to work from the comfort of your home in casual attire without the hassle of commuting. What a perk!

• Enjoy a consistent work schedule - Full-Time Monday through Friday, no weekends, with an average of once-per-year rotational on-call duty.

⛳️ Requirements

• Accepting transitions from the Patient Enrollment team to perform initial health assessments for newly enrolled patients in our network.

• Address patients' inquiries and foster an open dialogue to comprehend their needs.

• Identify and coordinate community resources that can enhance patient care.

• Deliver patient education and promote health literacy concerning the management of chronic conditions.

• Assist with medication management by identifying possible medication issues, ensuring adherence, and coordinating refills.

• Help ensure the timely delivery of services to your patients, including Home Health, DME, Home Infusion, and other essential needs.

• The candidate must be able to swiftly adapt to a fast-paced work environment, with most of the shift spent on the phone.

• Active Compact/Multi-State license (LPN) (LVN) is required.

• Possess technical proficiency in the Microsoft Office Suite.

• Strong written and verbal communication skills are essential.

• High-speed Wi-Fi is a must.

• A home office or HIPAA-compliant workspace is required.
