Intake Coordinator – Licensed Practical Nurse (LPN)

This is a fully remote position, open to applicants in Colorado.

📋 Description

• Receiving transfers from the Patient Enrollment team to perform initial health assessments for newly enrolled patients within our network.

• Addressing patients' inquiries and fostering open communication to better understand their needs.

• Identifying and coordinating community resources that would enhance patients' care.

• Offering patient education and promoting health literacy regarding the management of chronic conditions.

• Assisting with medication management, which includes recognizing potential medication issues, monitoring adherence, and coordinating refills.

• Helping ensure prompt delivery of services for your patients, including Home Health, DME, Home Infusion, and other essential needs.

• This role requires an individual who can swiftly adjust to a fast-paced work environment, with most of the shift spent on the phone.

• Valid Compact/Multi-State license (LPN) (LVN)

• Proficient technical skills – Microsoft Office Suite

• Outstanding written and verbal communication abilities
