📋 Description

• CareHarmony’s Intake Coordinators (LPN) (NLC) (LVN) collaborate extensively with providers to implement value-based care management initiatives for their patients.

• CareHarmony is on the lookout for a skilled Licensed Practical Nurse – LPN Nurse (LPN) (NLC) (LVN) with a minimum of 3 years of direct patient-facing experience, who thrives in a dynamic environment, is self-driven, possesses exceptional attention to detail, and appreciates the influence they can exert on a patient’s healthcare journey.

• You should have experience in identifying resources and coordinating necessities for chronic care management patients. This position is ideal for those who enjoy meeting and assisting new patients on a daily basis.

• Consistent schedule - Full-Time Monday to Friday, with no weekends, and an average of once-per-year rotational on-call duty.

⛳️ Requirements

• Welcoming transfers from the Patient Enrollment team to carry out preliminary health assessments for newly enrolled patients in our network.

• Address patients' inquiries and foster an open dialogue to comprehend their needs.

• Identify and coordinate community resources that would enhance patient care.

• Provide education to patients and promote health literacy regarding the management of chronic conditions.

• Assist with medication management, which includes identifying potential medication issues, ensuring adherence, and coordinating refills.

• Aid in the timely delivery of services to your patients, including Home Health, DME, Home Infusion, and other critical requirements.

• The candidate must be capable of quickly adjusting to a fast-paced work environment. This role necessitates being on the phone for the majority of your shift.

• Active Compact/Multi-State license (LPN) (LVN) required.

• Technical proficiency – Microsoft Office Suite.

• Exceptional written and verbal communication skills.

• Must have an active high-speed Wi-Fi connection.

• A home office or HIPAA-compliant workspace is essential.
