Intake Coordinator – Licensed Practical Nurse (LPN)

This is a fully remote position, open to applicants in Alabama.

📋 Description

• Accepting transfers from the Patient Enrollment team to perform initial health assessments for newly enrolled patients within our network.

• Address patient inquiries and foster open communication to better understand their needs.

• Identify and connect patients with community resources that can enhance their care.

• Offer patient education and promote health literacy regarding the management of chronic conditions.

• Assist with medication management, which includes identifying potential medication issues, ensuring adherence, and coordinating refills.

• Help guarantee the timely delivery of services to your patients, including Home Health, DME, Home Infusion, and other essential needs.

• This individual must be able to swiftly adjust to a high-paced work environment. The role requires spending most of your shift on the phone.

• Active Compact/Multi-State license (LPN) (LVN)

• Technical proficiency – Microsoft Office Suite

• Outstanding written and verbal communication skills

• Must possess active high-speed Wi-Fi

• Must have a home office or HIPAA-compliant workspace
