In-Home Traveler Nurse Practitioner – Physician Assistant

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Conduct in-home health assessments.

• Interact with individuals in their residences to help close care gaps.

• Travel up to 100% within a specified coverage area as required.

• Perform virtual consultations as necessary.

• Deliver all Signify Health services as specified, including Diagnostic & Preventive Services products.

• Possess an active, unrestricted license(s) in the applicable coverage area(s).

• Board certification is mandatory.

• Capability to obtain and sustain multi-state licenses in the coverage area, as needed.

• Willingness to serve all counties within the state and adapt to other business needs.
