In-Home Nurse Practitioner, Physician Assistant – Part Time

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Conduct health evaluations within clients' homes.

• Interact with individuals in the familiarity of their own environments.

• Establish trustworthy relationships to enhance individuals' health.

• Assess medications and review medical histories.

• Facilitate virtual consultations as necessary.

• Deliver all services offered by Signify Health as required.

• Possess an active, unrestricted license(s) within the designated coverage area(s).

• Hold board certification (mandatory).

• Multi-state licenses are preferred.
