In-Home Health Nurse Practitioner or Physician Assistant

This is a fully remote position, open to applicants in Maine.

📋 Description

• Perform In-Home Health Assessments

• Interact with individuals in their residences to help address care gaps

• Carry out virtual consultations as necessary

• Deliver all services provided by Signify Health, including Diagnostic & Preventive Services products

⛳️ Requirements

• Valid, unrestricted license(s) in the relevant coverage area(s)

• Board certification (mandatory)

• Multi-state licensure (preferred)

• Must be able to cover the following primary locations: Penobscot, ME
