This is a fully remote position, open to applicants in New Mexico.

📋 Description

• Connect members with suitable housing resources by collecting information, investigating available programs, and keeping resource files up to date.

• Ensure clear communication with members regarding their housing-related requirements.

• Create targeted resource lists and collaborate with community organizations.

• Stay updated on housing policies and changes in funding.

• Address housing referrals by gathering comprehensive details about the member.

• Investigate housing programs and resources in the member's locality and align them with appropriate options.

• Provide weekly reports on referrals handled and consult with colleagues regarding complex cases.

• Participate in rounds to offer housing resources as necessary.

• Present a curated list of housing resources to members and their Care Coordinators.

• Regularly update and maintain current housing resource files.

• Develop specific resource lists in response to emerging concerns or trends.

• Attend community events, meetings, and functions to gather additional resources and information about potential program changes.


⛳️ Requirements

• Minimum of 2 years of experience in housing or care management.

• A positive, recovery-oriented attitude that inspires hope and demonstrates empathy.

• Knowledge of housing resources, programs, and services, including eligibility criteria and application processes.

• Familiarity with federal, state, and local housing policies, funding, and procedures.

• Experience working with Medicaid populations, including individuals facing homelessness, older adults, or those with disabilities.

• Strong research and organizational skills to identify, maintain, and update resource databases.

• Ability to establish and maintain relationships with community organizations, agencies, and service providers.

• Excellent verbal and written communication skills.

• Problem-solving abilities to tackle unique housing challenges and identify appropriate resources.

• Proficiency in utilizing technology and the Microsoft Office Suite, including database management, spreadsheets, and communication tools.

• Capacity to attend and engage in community events, meetings, and functions as an organizational representative.

• Sensitivity to diverse cultural and socioeconomic backgrounds.

• Strong time management abilities and the capability to manage multiple priorities in a fast-paced environment.

• Experience in reporting and documentation, including maintaining detailed electronic health records and resource files.

• Flexibility to adapt to changing community needs and organizational priorities.

• Must possess a reliable vehicle for regular travel within the community.
