Hospice Triage RN, PT, Weekend

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Provide telephone triage for patients and families in hospice care.

• Evaluate patient conditions and identify suitable next steps.

• Work collaboratively with on-call teams to manage care and resources.

• Precisely document communications and interventions.

• Maintain a composed and professional attitude while managing urgent calls.

• Offer after-hours support and ensure timely interventions for patients receiving hospice services.

⛳️ Requirements

• Current multistate Registered Nurse (RN) license.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficient with technology and electronic medical records (EMR) for documentation purposes.

• Capable of typing documentation and notes in a fast-paced environment.

• Ability to manage stress and multitask while handling calls.

• Excellent communication and critical thinking abilities.

• Capacity to work independently in a remote setting.

• Dependable attendance and proactive communication during scheduled shifts.

• Stable internet connectivity during all scheduled shifts.

• Various opportunities to enhance the hourly rate, with the potential to reach up to $30 per hour within the first six months of employment.

• $2 shift differential for overnight shifts.
