Hospice Triage RN, PT

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Conduct telephone triage for hospice patients and their families.

• Evaluate patient conditions and identify suitable next steps.

• Work in partnership with on-call teams to coordinate care and resources.

• Ensure accurate documentation of all communications and interventions.

• Maintain a composed and professional demeanor while managing urgent calls.

⛳️ Requirements

• Valid multistate Registered Nurse (RN) license.

• Experience in hospice, palliative, or end-of-life care is highly desirable.

• Proficient with technology and electronic medical records (EMR) used for call documentation.

• Ability to type documentation and notes quickly in a fast-paced environment.

• Capacity to manage stress and multitask while handling calls (minimum of 5 calls per hour on weekdays, and up to 8 per hour on weekends).

• Excellent communication and critical thinking skills.

• Capability to work independently in a remote setting.

• This role is remote, requiring consistent attendance, active communication, and reliable internet connectivity during all scheduled shifts to facilitate timely patient care coordination.
