Hospice Triage RN, PT, 3p-12a, Rotating Sat & Sun 7:30a-4p CST

This is a fully remote position, open to applicants in Arizona.

📋 Description

• Conduct telephone triage for hospice patients and their families.

• Evaluate patient conditions and identify the appropriate subsequent actions.

• Collaborate with on-call teams to facilitate care and resource coordination.

• Carefully document all communications and interventions.

• Maintain a composed and professional demeanor while managing urgent calls.

⛳️ Requirements

• Current multistate Registered Nurse (RN) license is required.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficiency with technology and electronic medical records (EMR) for call documentation is essential.

• Ability to efficiently type documentation and notes in a fast-paced environment.

• Capacity to manage stress and multitask while handling calls (minimum of 5 calls per hour on weekdays, and up to 8 per hour on weekends).

• Excellent communication and critical thinking abilities are necessary.

• Capability to work independently in a remote setting is required.

• This remote position necessitates consistent attendance, proactive communication, and reliable internet connectivity during all scheduled shifts to ensure timely patient care coordination.
