Hospice Triage RN, PT, 3:30p-11p, Rotating Sat & Sun

This is a fully remote position, open to applicants in Florida.

📋 Description

• Deliver telephone triage services for hospice patients and their families.

• Evaluate patient conditions to identify appropriate next steps.

• Collaborate with on-call teams to ensure the coordination of care and resources.

• Document all communications and interventions with precision.

• Maintain a composed and professional attitude while managing urgent calls.

⛳️ Requirements

• Possession of an active multistate Registered Nurse (RN) license.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficiency with technology and electronic medical records (EMR) for call documentation.

• Capability and ease in typing documentation and notes in a fast-paced setting.

• Ability to manage stress and multitask effectively while receiving calls (minimum of 5 calls per hour on weekdays, and up to 8 per hour on weekends).

• Excellent communication and critical thinking abilities.

• Capacity to work independently in a remote setting.

• This position is remote and necessitates consistent attendance, active communication, and dependable internet connectivity during all scheduled shifts to facilitate timely patient care coordination.
