Hospice Triage RN, Full-Time, Monday-Friday

This is a fully remote position, open to applicants in United States.

📋 Description

• Facilitate telephone triage for hospice patients and their families.

• Evaluate patient conditions and ascertain suitable next steps.

• Collaborate with on-call teams to manage care and resources effectively.

• Document all communications and interventions with precision.

• Maintain a composed and professional demeanor while handling urgent calls.

⛳️ Requirements

• Currently possess an active multistate Registered Nurse (RN) license.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficient with technology and electronic medical records (EMR) for documenting calls.

• Ability to document and take notes efficiently in a fast-paced environment.

• Capability to manage stress and multitask while receiving calls (minimum of 5 calls per hour on weekdays, and up to 8 per hour on weekends).

• Excellent communication and critical thinking abilities.

• Capacity to work independently in a remote setting.

• This position is remote and necessitates consistent attendance, active communication, and dependable internet connectivity during all scheduled shifts for effective patient care coordination.
