Hospice Triage RN, Full-Time, Monday-Friday, 6:30am-4pm

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Conduct telephone triage for hospice patients and their families.

• Evaluate patient conditions and decide on suitable next steps.

• Collaborate with on-call teams to ensure effective care and resource coordination.

• Maintain accurate documentation of all communications and interventions.

• Uphold a calm and professional demeanor while managing urgent calls.

⛳️ Requirements

• Current multistate Registered Nurse (RN) license is required.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficiency with technology and electronic medical records (EMR) for call documentation is essential.

• Ability to type documentation and notes efficiently in a fast-paced environment.

• Capacity to manage stress and multitask while handling calls (minimum of 5 calls per hour on weekdays, and up to 8 per hour on weekends).

• Excellent communication and critical thinking skills are necessary.

• Must be able to work independently in a remote setting.

• This remote role requires consistent attendance, active communication, and reliable internet connectivity throughout all scheduled shifts to facilitate timely patient care coordination.
