This is a fully remote position, open to applicants in Oregon.

📋 Description

• Conduct in-home infusion therapy visits with patients following the established care plan.

• Consistently prepare clinical and nursing documentation, noting patient interventions in both clinical and non-clinical records.

• Serve as a clinical resource, collaborating effectively with other pharmacy departments.

• Stay informed about drug side effects and potential interactions.

• Provide both telephone and in-person education to patients regarding therapy protocols, drug side effects, and home infusion procedures.

• Ensure that patients comprehend their therapy plans, medications, and the significance of adherence.

• Function as a liaison during initial and ongoing consultations, facilitating communication between the patient, their family, and the pharmacy for a smooth therapy experience.

• Conduct thorough initial and periodic head-to-toe assessments for patients and initiate preventative and rehabilitative care as necessary.

• Utilize health assessment data, along with input from the agency team, physician, patient, and family to ascertain patient needs.

• Manage expectations of patients and families concerning services, outcomes, and discharge goals.

• Establish primary and secondary diagnoses based on patient evaluations and home health care objectives.

• Create care plans that include skilled interventions, medical supplies, and necessary services to achieve set goals.

• Regularly assess patient progress in collaboration with team members and modify care plans as required.

• Perform additional duties as assigned.


⛳️ Requirements

• Graduate from an accredited nursing school with a valid RN state license.

• A minimum of 2 years of experience in home infusion and infusion centers.

• At least 2 years of experience with IV Immunoglobulins and IV Biologics.

• CRNI certification (Certified Registered Nurse Infusion) is preferred.

• Preferred experience with peripheral IV starts, infusion therapy, and phlebotomy for both adult and pediatric patients.

• Strong interpersonal, time management, and organizational abilities.

• Excellent written and verbal communication skills.

• Organizational capability to maintain consistently accurate records.

• Ability to direct information to the appropriate healthcare provider.

• Proficient in computer skills, including word processing, and efficient use of the internet and email.

• Current CPR and BLS (Red Cross and AHA) certification, valid driver’s license, reliable transportation, and automobile insurance.
