Home Health Aide, Certified Nursing Assistant

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Implement physician-ordered care plans and ensure accurate documentation in the record-keeping system.

• Deliver prescribed treatments and therapies.

• Conduct patient evaluations and coordinate overall care.

• Support patients with daily living activities, communication, and monitoring of vital signs within their homes.

• Offer individualized care in the comfort of patients’ residences.

• Liaise with family members, physicians, and other designated individuals about patients’ health status.

• Provide personal care services as required to fulfill patient needs.

• Organize scheduling in collaboration with the HHA/CNA, patient, and case manager.

⛳️ Requirements

• Valid CPR certification.

• Valid First Aid certification.

• Current 2-Step TB Test.

• Preferred experience in a clinical care setting; home health experience is a plus.

• Strong interpersonal skills.

• Capability to stand and walk for extended durations.

• Ability to lift up to 50 lbs. and assist with patient mobility.

• Good hand-eye coordination and manual dexterity.

• Must be able to pass a background check.
