Home Care & Hospice Virtual Care Center Coordinator, RN

This is a fully remote position, open to applicants in Florida.

📋 Description

• Perform virtual assessments and oversee patients' health conditions remotely.

• Create and coordinate personalized care plans in partnership with the interdisciplinary team.

• Offer virtual education and support to patients, families, and caregivers regarding symptom management and end-of-life care.

• Collaborate with field nurses, physicians, social workers, and other team members to ensure comprehensive care delivery.

• Quickly respond to urgent virtual consultations and effectively triage patient needs.

• Leverage telehealth technology to conduct regular check-ins and follow-ups with patients and families.

• Provide telehealth hospice care and offer real-time guidance to field staff and caregivers.

⛳️ Requirements

• Over 5 years of experience in Home Care, Hospice, ER, and/or Triage as an RN.

• Current RN license in a Compact State (eNLC).

• Bachelor of Science in Nursing (BSN) from an accredited nursing education program.

• Familiarity with Epic or experience with Home Care EMR.

• Excellent communication and interpersonal skills to build rapport with patients and families virtually.

• Ability to work independently and as part of a collaborative remote team.

• Capable of adapting to rapid changes to meet business or departmental needs.

• Experience in patient or customer service.
