This is a fully remote position, open to applicants in New York.

📋 Description

• Manage home care referrals and submit clinical information to the home care agencies.

• Gather clinical data from the EMR to provide to the home care agency, facilitating a smooth transition of care.

• Serve as a liaison among inpatient staff, physician offices, and home care agencies to ensure effective communication regarding home care orders and the medical needs of patients.

⛳️ Requirements

• Associate's Degree in Health Care, Business, or a related field.

• Minimum of 1 year of customer service experience is required, preferably within the healthcare sector.

• Proficiency in entering data, as well as navigating and retrieving information from an electronic medical record (EMR), such as Epic or Careport Care Management.

• Experience in home care placements and/or home care intake is highly desirable.

• Familiarity with managed care Medicare, Medicaid, or other insurance authorization, along with an understanding of CMS regulations related to home care services, is preferred.

• Exceptional verbal and written communication skills.

• Strong orientation towards customer service.

• Ability to adapt to changes and work effectively in a team-oriented environment is required.

• Capability to manage multiple priorities.
