This is a fully remote position, open to applicants in North Carolina.
• Identify residential and other provider agencies for members enrolled in Tailored Care Management.
• Collaborate closely with the Primary Care Manager and Regional Care Teams to find suitable residential and additional supports based on the member's care needs.
• Complete necessary documentation, paperwork, and tasks in a software platform in accordance with required timelines.
• Facilitate integrated care management through multi-disciplinary collaboration and referrals.
• High School Diploma or GED, along with a minimum of one (1) year of experience working with individuals with Intellectual Developmental Disabilities (IDD), Traumatic Brain Injury (TBI), Behavioral Health/Mental Health/Substance Use (BH/MH/SU), or Innovations populations; or an equivalent combination of education and experience.
• A valid driver’s license is mandatory.
• Residency within Trillium’s catchment area is required, which includes the North Carolina counties: Bertie, Camden, Chowan, Currituck, Gates, Halifax, Hertford, Martin, Northampton, Pasquotank, Perquimans, Warren, Beaufort, Dare, Edgecombe, Greene, Hyde, Nash, Pamlico, Pitt, Tyrrell, Washington, Wilson, Carteret, Craven, Duplin, Jones, Lenoir, Onslow, Pender, Sampson, Wayne, Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, Scotland, Anson, Guilford, Montgomery, Randolph, and Richmond.
