Health Services Coordinator

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Acts as the primary contact for newly referred patients, playing an essential role in patient outreach, registration, enrollment, and continuous telephonic engagement.

• Assists patients dealing with chronic illnesses such as congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), diabetes, and hypertension.

• Tasked with establishing trust, showing empathy, and effectively communicating Remote Patient Monitoring (RPM) services.

• Works in close collaboration with nurse care managers and other members of the clinical care team to help patients achieve their health objectives and comply with care plans.

• Conducts necessary telephonic interactions, which include Welcome Calls and Post-Physician Visit Updates.

⛳️ Requirements

• A minimum of six (6) months of experience in general duties as a Medical Assistant OR Three (3) years of experience in a clinic or similar healthcare environment with direct patient interaction.

• Familiarity with medical terminology and prior experience using electronic health record (EHR) systems.

• Strong technical skills, including the capability to assist patients remotely with health devices and resolve basic issues.

• Ability to type at least 50-60 words per minute while conversing with patients over the phone.

• Must be available to work schedules that align with client-specific time zone requirements, as determined by business needs.

• Active CNA certification or an equivalent certification (e.g., Pharmacy Technician), in good standing.
