Health Engagement Specialist

This is a fully remote position, open to applicants in Alaska, +1 more state.

📋 Description

• Make numerous outbound calls to patients, including those who are new to Diverge Health.

• Handle incoming calls from patients responding to outreach from Diverge Health or referrals from their primary care providers, as well as from primary care providers coordinating patient services with Diverge Health.

• Identify and assist patients who frequently use the emergency room by directing them toward more suitable care options.

• Reach out to patients experiencing care transitions (e.g., from hospital to home post-discharge) to confirm they have follow-up care arranged, and educate them on the significance of preventive care and regular visits with their healthcare provider.

• Aid patients in booking appointments, exploring care options, and overcoming access challenges.

• Establish trust with patients and customize conversations to address their specific needs.

• Keep precise and organized records of patient interactions utilizing CRM systems (e.g., Salesforce).

• Achieve or surpass daily outreach and engagement performance targets.

• 1-2 years of experience in patient engagement, outreach, or customer service within a healthcare environment.

• Excellent communication and interpersonal skills, with a proven ability to build trust rapidly.

• A strong sense of empathy and compassion for patients from various backgrounds.

• Ability to thrive in a fast-paced, dynamic environment with changing priorities.

• Problem-solving mindset and critical thinking skills for addressing patient barriers to care.

• Knowledge of care coordination, preventive services, and transitions of care (preferred).

• Experience with CRM systems (e.g., Salesforce) and call management tools, including basic administration of these systems.

• Strong organizational skills and the capacity to manage multiple tasks concurrently.
