This is a fully remote position, open to applicants in Massachusetts.
• Transform the delivery of clinical care to patients in primary care as a Health and Wellness Coach (HWC) / Virtual Group Visit Coordinator (VGVC).
• Collaborate closely with primary care providers and patients to establish behavior change goals within both group settings and individual consultations.
• Follow up with patients, distribute educational resources, collaborate on workflows, and assist with appointment scheduling.
• Enhance physician- and nurse practitioner-led Virtual Group Visits (VGVs) with tailored coaching plans.
• Build and maintain ongoing relationships with patients to support their journey in achieving behavior modifications.
• Facilitate and support the execution of Virtual Group Visits (VGVs).
• Engage with patients to boost their involvement and provide health updates.
• Bachelor's Degree in a Related Field of Study: Required
• National Board Certified Health and Wellness Coaching (NBC-HWC) Certification: Required
• Experience in health promotions or educational settings, counseling on health topics such as nutrition, exercise, cholesterol, hypertension, stress, and diabetes.
• 2-3 years of experience preferred
• Exceptional customer service skills.
• Excellent listening abilities.
• Outstanding communication skills across all formats - virtual, phone, electronic, and in-person.
• Strong clinical and analytical skills in evaluating patient health needs, encompassing medical, social, and behavioral health components.
• Capacity to multitask and adjust priorities to best serve the needs of patients and teams.
• Proven ability to manage sensitive information with discretion.
• Demonstrated capability to work independently and collaboratively within a team.
• Proven performance under pressure and in a dynamic environment.
• Self-motivated; able to organize and prioritize tasks effectively.
• Ability to innovate, think creatively, and enhance processes.
• Proficient with Zoom and Microsoft Office desktop applications; familiar with database management software.
• Competent in using the Internet for research purposes as necessary.
• Proven level of professionalism to interact effectively with all levels of administration.
• Basic understanding of chronic diseases, particularly hypertension, hypercholesterolemia, diabetes, and pre-metabolic syndrome.
• Knowledge of fitness and nutrition is preferred, and the potential to collaborate with a nutritionist and physical therapist for care plans.
